Thank you for your interest in receiving services from Access Allied Health & Training. Please note we currently prioritise swallowing assessments for adult and Parkinson's disease voice programs and therapy for progressive neurological conditions. Complete the referral information (which is the commencement of intake only) below. Once received will be in touch to book your appointment with us.  Case history, reports and letters from other clinicians are required enabling us to provide you with holistic care. You are also required to agree to our "Service Agreement" for services. If you are NDIS plan managed ensure a "service request" is provided for therapy to commence from your coordinator. Please send relevant reports to info@accessalliedhealth.net.au so we can better service your needs.
